Patients
Your First Visit
Please bring your referral form, picture ID and insurance card.
Please bring co-pay, if applicable. We accept cash and all major credit cards.
Please bring any pertinent medical records you feel your referring physician may not have had access to at the time of scheduling.
For your convenience and to speed-up your check-in process, you may fill out and e-sign your patient forms prior to your visit. Please print out forms and bring them to your appointment. You will need Adobe Acrobat Reader to open these electronic forms.
